Based on the CBCT, I was only expecting to find an untreated MB2. Pleasant surprise in this one — additional untreated MB3 and MB4 😳

Necrotic 41/31 (with acute apical abscess), likely

Necrotic 41/31 (with acute apical abscess), likely secondary to trauma sustained years ago. Severe crowding along the lingual of the 41 necessitated an incisal access. Important to keep the preparations of mandibular incisors as conservative as possible to minimize risk of VRF in the longer term. Let’s see how this one heals in 12m. 🤞

Regenerative endodontic procedure (revasvularizati $Regenerative endodontic procedure (revasvularization) on an immature mandibular premolar. Patient initially presented with a necrotic pulp and acute apical abscess, secondary to a dens evaginatus (Pic 1). Following endodontic access and 3 week Ca(OH)2 placement, bleeding was induced in the apical half of the root and bioceramic putty was placed to seal the coronal half (Pic 2). Access was immediately sealed with composite resin. AAE’s regenerative endodontic procedure protocol was followed for this one. 1 year follow up (Pic 3) shows apical closure and continued root formation. The periapical lesion also appears to have healed. 2 year follow up (Pic 4) shows further root development. Note the calcification of the root canal system. Calcification is a common outcome with these regenerative endodontic cases. This will make any future RCT more challenging; however, the benefit of further root development and strengthening (i.e. reducing the risk of root fracture) will certainly prolong the lifespan of this tooth. Definitely a net positive!$

Remarkable 6-month healing on this severely calcified 37. Still have no idea why this one went necrotic — no cracks or history of trauma. Could be ortho-related 🤷🏻‍♂️. Doesn’t really matter — endo works! 🤩

Tooth 24: Root end surgery (apicoectomy). Retreatment is primarily the treatment of choice for failing RCTs, but given the coronal obstructions (crown and a palatal post), apicoectomy was the less invasive approach in this case.

Retreatment in a heavily restored tooth like this one can be undertaken, but there’s a fair probability that the crown could dislodge and tooth could be deemed non-restorable following post removal.

Treatment rendered: Apicoectomy, apical curettage, and placement of bioceramic retrofillings in the 2 root ends to seal the apices.

Next up: Tooth 26 retreatment. Lesion noted at the MB root end. Can you guess why this lesion has formed? (Hint: there’s almost always an MB2…! 😎)

12 month recall. Tooth 36. Incredible complete healing of this (very) large combined endo/perio lesion. 🤩 Thankfully extruded bioceramic sealer is well-tolerated by the PA tissues! 😅
Crown recommended ASAP to better seal that distal margin!!

When it comes to mandibular incisors, always look for that deep splitting lingual canal 😉

4 year recall on teeth 17/15/14. All 3 teeth were

4 year recall on teeth 17/15/14. All 3 teeth were necrotic and RCT’d simultaneously in 2020.
Today’s recall: we can see remarkable healing of the combined perio-endo lesion along the 17 mesial (as can be seen on the CBCT slice). Teeth 15/14 healed nicely too. Also very interesting to see the change in bony architecture around the 16 implant (following sinus lift).

What a difference 4 years makes! 👌
